Documentation of self-designated AMA PRA Category 2 Credit™

CME Record prepares a complete file for a CME office: physician identity, activity description, dates and location of participation, hours claimed, signed attestation, and an itemized receipt. It is a documentation service. It does not accredit activities or award Category 1 credit.

For CME administrators

CME Record does not certify continuing medical education and is not accredited by the ACCME. Each file is the physician’s own self-designation of AMA PRA Category 2 Credit™ under the American Medical Association Physician’s Recognition Award credit system.

A completed file includes the activity title and type, dates and location of participation, hours claimed in 0.25-hour increments, the physician’s attestation to AMA Category 2 criteria, and an itemized receipt. Acceptance for licensure, credentialing, or employer reimbursement is determined by the receiving organization.

What this service provides

Formatted documentation of a physician’s self-designated Category 2 activity: cover letter, self-designation record, and itemized receipt, including dates, location, and optional lodging.

What this service is not

Not an ACCME-accredited provider. Not a Category 1 certificate. Not a course, lecture series, or live seminar. Credit is self-designated by the physician, as the AMA PRA system requires for Category 2.

What a receiving office can verify

Physician name and credentials, activity description, dates and place of participation, hours claimed, signed attestation, and the documentation fee. The office applies its own reimbursement policy.

How it works

From completed learning to a file a CME office can review.

01

Complete the learning

Category 2 activities are self-directed: literature, teaching, peer consultation, writing, quality review, and other qualifying work related to the physician’s practice.

02

Record and attest

Enter specialty, activity, hours (0.25 increments), dates, and location. Sign the AMA Category 2 criteria. The file is generated immediately.

03

Submit the file

Print or save as PDF. Provide the cover letter, self-designation record, and receipt to the CME office that administers reimbursement or credit tracking.
